1. Learn your state’s rules first

NEMT is regulated state by state. Your state Medicaid agency and department of transportation decide what licenses, vehicle inspections, and driver checks you need. Read their provider requirements before you buy anything.

2. Form the business and get insured

  • Register an LLC or corporation and get an EIN.
  • Get commercial auto liability, general liability, and workers’ compensation. Many payers set minimum coverage amounts.
  • Open a business bank account and set up bookkeeping from day one.

3. Choose your vehicles

Many operators start with one or two wheelchair-accessible vans, since wheelchair trips are in steady demand and often pay more than ambulatory rides. Make sure every vehicle meets your state’s equipment and inspection rules.

4. Hire and train drivers

Drivers typically need background checks, drug screening, a clean driving record, and training in CPR, first aid, passenger assistance, and wheelchair securement. Keep every certificate on file with its expiration date.
